IVF is a fertility treatment in which a woman’s egg and a man’s sperm are combined in a laboratory in order to achieve fertilization.The embryo(s) is then transferred to the uterus.After 2 weeks of implantation, pregnancy test performs with the hope of IVF success.

There are five basic steps in the IVF and embryo transfer process

Step 1: - CONSULTATION

  • You meet with our IVF experts and based on your history, examination, ultrasound and blood reports the best and most suited IVF protocol will be tailor-made especially for you 

STEP 2: - OVARIAN STIMULATION

  • Fertility medications are prescribed to stimulate egg production. Multiple eggs are desired because some eggs will not develop or fertilize after retrieval. A transvaginal ultrasound is used to examine the ovaries.
  • Injections have to be taken approximately at the same time every day. It is advisable to come to the hospital for your injections, but if you can't, our medical staff will teach you how to self-administer the injections or you can have them taken locally by a doctor.

STEP 3 : -hCG injection/GnRH injection

  • This is given to trigger the final maturation of the eggs and make them ready for collection, which happens about 34 to 36 hours later.

STEP 4: - OVUM PICK-UP

  • Eggs are retrieved through a minor surgical procedure that uses ultrasound imaging to guide a hollow needle through the pelvic cavity to remove the eggs. Medication is provided to reduce and remove potential discomfort. The procedure takes 15-30 minutes and the number of eggs collected will depend on your response to the injections. You can go home approximately 2-3 hours after the procedure

STEP 5: -SPERM RETRIEVAL

  • Usually, a fresh semen sample is collected on the day of ovum pick-up (egg collection). Some men find it difficult to produce a semen sample on request, so it may be collected elsewhere and brought to the center within 30 minutes of collection. Generally, to produce the best sample, abstinence of 3-5 days is recommended and lubricants should not be used. 
  • However, if a fresh sample cannot be produced on the same day or if the husband is not available on the day of egg collection, a previously frozen sample can also be used

STEP 6:-FERTILIZATION BY IVF/ICSI

  • In a process called insemination, the sperm and eggs are mixed together and stored in a laboratory dish to encourage fertilization. In some cases where there is a lower probability of fertilization, intracytoplasmic sperm injection (ICSI) may be used. Through this procedure, a single sperm is injected directly into the egg in an attempt to achieve fertilization.  The eggs are monitored to confirm that fertilization and cell division are taking place. Once this occurs, the fertilized eggs are considered embryos.

STEP 7:-EMBRYO TRANSFER

  • The embryos are usually transferred into the woman’s uterus three to five days following egg retrieval and fertilization. A special catheter is inserted into the uterus to transfer the embryos. This procedure is painless for most women, although some may experience mild cramping. After the transfer, you will be given a course of medication to support the luteal phase. 
  • If there are excess good quality embryos, it is possible to freeze them for later use.

STEP 8 : -PREGNANCY TEST

  • If the procedure is successful, implantation typically occurs around six to ten days following egg retrieval. About two weeks after the embryo transfer, you will be given a beta-hCG test to check if gestation has been achieved.  If the test is positive, a vaginal ultrasound scan will be performed 1 week later to confirm the pregnancy and view the gestational sac.
  • Damaged or blocked fallopian tubes: When fallopian tubes are damaged or blocked, IVF is the only option recommended if you need a biological child. Fallopian tubes are important, these attach the ovaries with the womb. If the egg ovulated from your ovaries and can’t reach the womb, you can’t conceive.
  • PCOS (polycystic ovarian syndrome): The patients associated with PCOS often conceive less. IVF treatment is highly recommended and performs amazingly for the patients.
  • Recurrent miscarriage: More and more couples who are distressed with losing their baby repeatedly are choosing IVF especially with pregenetic testing to increase their chance of live birth and to decrease

  • Advanced maternal age/ menopausal women

  • Male infertility: In the case of very low sperm count /defective sperm in males, IVF provides the best treatment. In IVF sperm are placed together with the eggs in a petri dish and are therefore not required to swim through the cervix, uterus, and into the tube to fertilize the egg.
  • Unexplained infertility: IVF may also be recommended for a patient with unexplained infertility who has undergone several unsuccessful attempts with fertility medications and intrauterine insemination (IUI)
  • Genetic abnormalities in either partner: If a couple experiences frequent miscarriage due to genetic disorders, IVF is the only option to get a successful pregnancy.

  • Ovarian hyperstimulation syndrome (OHSS): Ovarian hyperstimulation syndrome is an exaggerated response to excess hormones. It usually occurs in women taking injectable hormone medications to stimulate the development of eggs in the ovaries. Ovarian hyperstimulation syndrome (OHSS) causes the ovaries to swell and become painful. Symptoms may range from mild abdominal pain, bloating, nausea, or vomiting to decreased urinary frequency, shortness of breath, faintness, and Severe stomach pains.

  • Multiples pregnancy - There are additional risks and concerns related to multiples during pregnancy including the increased risk of premature delivery and low birth weight.

Though the rates of miscarriage are similar to unassisted conception, the risk does increase with

maternal age.

  • Ectopic pregnancy- The risk of ectopic pregnancy with IVF is 2-5%. An ectopic pregnancy is when a fertilized egg implants anywhere outside the uterus and is not viable.

Precautions after an IVF treatment?

  • After embryo transfer generally, the patient is advised to avoid vigorous exercise, heavy lifting, jerky

traveling and other activities that involve potentially jarring movements (cycling, jogging, skiing, tennis etc.).

  • All routine day to day activities are allowed.
  • Take medications as prescribed by the treating doctor. If you become pregnant you might be asked to continue some of these medications until the 12th week of pregnancy when the body usually starts producing enough.
  • Avoid smoking, exposure to cigarettes, alcohol, and exposure to X-rays.
